How to join the Practice

REGISTERING WITH THE PRACTICE 

Patients who have moved into our practice area are eligible to register with the practice. Please see here for our Practice Boundary.

Unfortunately the Practice is currently only accepting a limited number of new patients each week, who do not already have a local GP. This restriction has been necessary to ensure that our existing patients continue to receive a safe and good quality service from our practice.

If you wish to register with the practice you must hand in your forms in person on a Wednesday morning at 8am, please be aware however we  are only able  to register a limited number of patients each week . We will not accept registration forms via email or post. Please do not post your forms through the Practice letter box. 

The Practice reaches the maximum patient registrations on a regular basis. You may be told that the maximum registrations for that week has been reached and be asked to return again the following week. We apologise for any inconvenience.

To join the practice list you will need to completed and signed the following THREE forms: 
  • a registration form which can be downloaded and completed here Please remember to sign this form.  
  • a New Patient Questionnaire which can be downloaded here. This will provide us with information about your medical and family history until we receive your notes from your previous practice. If you have a child under 5 years old please download this form here instead. 
  • a Data Protection Form which can be downloaded here.  This will give consent for someone else to contact the practice on your behalf. You can also give consent to SMS texting.
When handing in these forms you will also need to provide:
  • Proof of identity - preferably something with photographic ID e.g. passport, driving licence etc
  • Proof of address e.g a utility bill, lease agreement etc.
  • If you are a non-British Citizen, we will  also need to see your Visa. 

If you take any medication we would ask that you ensure you have a sufficient supply of your medicine from your current Practice to last at least 2-3 weeks before registering with our Practice. This will avoid running out of your drugs before the Practice has a chance to register you and provide a new prescription.

Please be aware that if you take regular medication registration forms cannot be accepted without an attached repeat reorder medication slip or with the medication section of the new patient questionnaire form completed. 

Please allow 48 hours if possible for us to complete your registration before arranging an appointment. 

We do not register patients who live outside our Practice boundary area and any patients who subsequently move out with our boundary will need to register with a new GP practice.
